Unity HMO Members

I am an approved Unity massage provider. If you are a Unity HMO member, you may be reimbursed through Unity for massages received from me (annually up to $200 for family plans and $100 for individual plans). Please refer to the Unity website for clarification about eligible expenses for the Fitness First & More program and the distinction about how individual and family plans are treated differently.

Follow the instructions below for reimbursement.

  1. Go to https://unitymychart.com/mychart/
  2. Login to your Unity MyChart account. If you don’t have a My Chart account yet, you can request one from this page.
  3. Under the My Insurance tab, select Wellness Programs
  4. Select the Fitness First & More Reimbursement Form button
  5. Fill out the form online -click Integrative Medicine box -click the Massage/Therapy and Bodywork box -click the Unity Health First Vendor box -fill in Create Thrive Grow in the space to the right of Unity Health First Vendor -fill in the total of your massage receipts in the Amount Paid for Service space
  6. Print the form
  7. Attach your massage receipts from Create Thrive Grow
  8. Mail the reimbursement form and receipts to: Unity Health Insurance Attn: Fitness First & More 840 Carolina Street Sauk City, WI 53583

NOTES:

All 2017 reimbursement requests must be received by January 15, 2018.
